HAZWOPER Certificate Requirements

In accordance with OSHA regulations 29 CFR 1910.120, the following elements are required for a HAZWOPER Certificate. This information was obtained from 29 CFR 1910.120 Appendix E (Non-Mandatory Training Guidelines).

Name and signature of instructor
Date the course was taken
Address of trainer provider
Student's name
Title and regulatory citation of course
Certificate number
Statement that a student has successfully completed the course
List of the levels of personal protective equipment used by the student to complete the course
Note: A student signature in lieu of an instructor signature is not permitted
